EOPS/CARE APPLICATION
By completing this application you may be eligible to receive EOPS/CARE services. EOPS/CARE applications are accepted every semester (Fall, Spring and Summer). You may download the application, complete it and then take it to the EOPS/CARE office.

MIDTERM PROGRESS REPORT
Will be available in March 2013

Note for Internet classes: simply print out your up-to-date Gradebook and attach to your Progress Report. An email from your instructor is required only if your Gradebook is NOT up-to-date.

PRIORITY REGISTRATION CHECKLIST
Use this checklist for help in the registration process. Explained are both the Internet Registration (PAWS) and the Phone Registration (ASAP) options.

This exit survey is intended for students who are no longer eligible to receive EOPS/CARE services. If you are a current EOPS/CARE student and would like to offer feedback, please download this anonymous form, complete it and submit it to the EOPS/CARE office.
